Note: Please read "How to place items on reserve" before completing the Reserves Request Form.
Note: The title should reflect what's on the syllabus.
Photocopied materials for electronic reserves will not be returned. If you want to pick up your material, it must be picked up before the end of the semester.
I have placed personal items on reserve at EKU Libraries with the understanding that these items may be accidentally mutilated, damaged, or lost. I hereby release the Library from all responsibility in replacing any personal materials. I also give the Library permission to process my personal materials as needed for reserve use [barcodes, pockets, labels, etc.]. By signing below, I acknowledge that I understand the terms above. I have read the fair-use statement provided in the directions and I attest that the materials that I have submitted for reserve are in compliance with copyright law.
Electronic copies of copyrighted materials added to Blackboard Course modules must not be used in subsequent semesters without receiving or purchasing copyright permission. This permission should be requested through EKU libraries. For more information about requesting the purchase of copyright permissions please contact Savannah Marlow at savannah.marlow@eku.edu or (859) 622-1790.
If you have any questions please e-mail Jens Arneson at jens.arneson@eku.edu.
